Patrick Muldoon's memorial service has taken place, almost a month after his death.

The former Melrose Place actor passed away on April 19 at the age of 57 after suffering a heart attack in the shower at home and on Saturday (16.05.26), his loved ones gathered at St. Monica's Catholic Church in Santa Monica to say their final farewells to the star.

Patrick's former USC Trojans teammate Rodney Peete wrote on Instagram: "Tough Day saying goodbye to my friend Pat Muldoon!!! He was my Teammate, My Brother, My Friend[prayer emoji] miss you already brotha.(sic)"

OnlyFans model Sami Sheen - whose mom Denise Richards previously dated Patrick - shared the accompanying handout on her Instagram along with the caption: "Miss u so much Patrick".

The front of the flyer bore the words, "Celebration of the spirit of Patrick Muldoon" over a photograph of the actor wearing a suit, with the church details and date underneath.

The Days of Our Lives star's death certificate was released at the end of April and revealed his remains had been cremated on 28 April.

The document also confirmed he had passed away due to myocardial infarction - a heart attack - with pulmonary embolism and hereditary coagulopathy listed as underlying causes.

A pulmonary embolism is "when a blood clot blocks a blood vessel in your lungs", according to the National Health Service (NHS).

Coagulopathy, meanwhile, "happens when there's a problem with how your blood forms clots", Cleveland Clinic states.

Sami, whose father is actor Charlie Sheen, had previously shared her sadness over Patrick's death.

Sami took to her Instagram Story to share photos and video clips of Denise and Patrick and wrote: "Patrick was someone that everyone loved. he was so quick witted and humble but still made sure everyone knew he was a rockstar and never cared what people thought about him. from day 1 till the last day I saw him, he's never changed.

"I've been lucky enough to grow up with him and i have yet to live my life without him. the friendship him and my mom had was so beautiful + something not many people get to experience.(sic)"

Sami insisted Patrick will be "missed every single day".

She concluded: "I hope i wake up from this nightmare soon."

Sami then shared another Instagram Story featuring what she felt was a message from Patrick.

Alongside a photo of a heart-shaped cloud in the sky, she wrote: "Right after he passed i was sitting outside begging for a sign from him to let us know that he's okay and made it to the other side, and then a couple minutes later i looked up and saw this in the sky. can't be a coincidence.(sic)"

Denise and Patrick first met in an acting class in the 1990s before starring together in the 1997 film Starship Troopers.

The pair dated for nearly five years before remaining close friends and collaborators, most recently working together on the upcoming crime thriller Dirty Hands.
